It all began with a seemingly innocuous mole the size of a 20-cent coin. The mole bled whenever Peter (not his real name), 56, lay down but it did not hurt. So he thought nothing of it and simply applied off-the-counter medicated oil in hopes of stopping the bleeding, which went on for a month. But the blood stains on his shirt worried his wife, who persuaded him to seek medical help in August last year.He was diagnosed the most dangerous form of skin cancer, at the National Skin Centre and was referred to the National Cancer Centre Singapore (NCCS).Peter, who has a son in his 20s, underwent surgery to remove the affected tissues surrounding the mole. He was in pain, could barely walk and had bad appetite.Earlier this year, eight months after the surgery, Peter suffered a relapse of the skin cancer.He experienced symptoms such as diarrhoea and swollen knees. He was told that the melanoma had spread to other parts of his body. Now, Peter takes Keytruda, the latest advanced melanoma drug to be approved by the Health Sciences Authority. Cord blood an important source of stem cells, with diseases such as blood cancer on the rise
More mothers are donating their newborns' cord blood, going by the rising numbers registered by the public cord blood bank. Last year, the Singapore Cord Blood Bank (SCBB) received 3,927 cord blood donations - a 67% jump from the 2,356 in 2010. This year, more than 3,000 donations were received as of Nov 30. While this is encouraging, the SCBB is aiming to do more, said its chief executive, Mrs Tan-Huang Shuo Mei. The bank stores about 11,000 cord blood units now. It is working towards reaching 15,000 units in five years, she said. The need to ramp up is pertinent, given that diseases such as blood cancer are on the rise, said Dr William Hwang, the SCBB's medical director. Based on the SCBB's current inventory, there is an 80% chance of finding a suitable cord blood match for Chinese patients. For Malays, the possibility is about 30 - 40% and Indians, less than 20%. At S'pore GH, outpatient chemotherapy administered for blood cancer numbered about 5,000 cycles in 2005. This grew to about 20,000 cycles in 2013. In some cases, when chemotherapy fails, or when the patient is genetically predisposed to a relapse, cord blood enters the picture. It is an important source of stem cells which can develop into healthy blood-forming cells in the patient's body. Based on the SCBB's current inventory, there is an 80% chance of finding a suitable cord blood match for Chinese patients. For Malays, the possibility is about 30 to 40% and Indians, less than 20% Moreover, a patient's chances of a match can become even slimmer if he requires more than one cord blood unit, said Dr Hwang. Both cord blood units have to match each other, and these two units have to match the patient. An adult typically needs two cord blood units while a child usually requires one. If more parents do not donate their babies' cord blood, doctors will have problems finding the right cord blood for patients, said Dr Yeap Min Li, an obstetrician and gynaecologist in private practice. The SCBB is the only public cord blood bank in Singapore. It depends on philanthropic support for its cord blood collection, cryopreservation and storage activities. The units are made available to anyone in need. The cost of services like retrieval and testing of the cord blood are paid by the recipient's family. But if a person needs his previously donated cord blood for himself and it is still in the bank's inventory, it will be released without any fee - if the transplant physician has advised that the unit will be suitable for his treatment. There are also 3 private cord blood banks here which keep the donor's cord blood exclusively for a family's use, for a fee. Launch a decade ago, the SCBB has facilitated 183 cord blood transplants, mostly to save people with blood-related conditions such as leukaemia and sickle-cell disease. The procedure involves a transfusion of donor's cord blood into the patient, who then has to take immunosuppressive drugs for six months to a year. When a match cannot be found here, doctors may turn to foreign cord blood banks. Dr Anselm Lee, medical director, said it uses one or two foreign cord blood units every year. While these patients can also be helped by a bone marrow transplant. Dr Lee said: "For patients with advanced leukaemia, time can run out. There are times when we have to hold on to a more reliable stem cell source for a transplant, instead of trying to wait for the best donor". Despite the life-saving benefits of cord blood, only about 9% of mothers today donate their babies' cord blood, said Mrs Tan-Huang. "It is perhaps not as well-known, the service is available at all public and private obstetric hospitals." Regardless of whether parents decide to opt for public or private cord blood banking, the bottom line is that the blood should not be wasted. "Hopefully, in the future, no cord blood will be discarded," said Dr Hwang. "It is such a precious resource." |

The study was done on human breast cancer cells growing in lab dishes, and it' s unclear whether these chemicals, called parabens, act the same in the human body. But the chemicals have been shown in previous lab and animal studies to mimic the activity of  the hormone estrogen, meaning they can bind to receptors in the body to which estrogen normally binds.This is a concern because when estrogen binds to estrogen receptors, it causes cells to multiply, and in women this increases breast cancer risk, said Dr. Dale Leitman, an adjunct professor of nutritional sciences and toxicology at the University of California, Berkeley, and the senior author on the study. Parabens are a type of chemical preservative, and are found in a wide array of consumer products, including shampoos, body lotions and sunscreens. It' s also known that parabens can activate the same pathway as estrogen, but previous studies have found that they do so very weakly, Leitman told Live Science. " Because they' re weak, they' re assumed to be safe compounds," especially based on the levels of parabens that have been found in humans, he said. [What Is Estrogen?] But previous studies looked at just the parabens by themselves, Leitman said. " The real problem when you do studies in the laboratory is that you study one compound at a time, but in the body, that' s not the case. What you' re seeing in the body is really a combination" of the effects of many compounds, Leitman said. In the new study, published today (Oct. 27) in the journal Environmental Health Perspectives, the researchers focused on the effects of parabens when mixed with one additional compound: a type of growth factor called heregulin that has also been linked to breast cancer cell growth. In experiments, the researchers looked at how well the cells grew when they were exposed to both parabens and heregulin, compared with how the cells grew when exposed only to parabens. The scientists found that when they added heregulin, they could drop the level of parabens by 100 times and the cancer cells would still multiply faster than those without heregulin. In other words, when the heregulin was added, the parabens were 100 times more potent at stimulating cancer cell growth compared with breast cancer cells exposed to only parabens. With this increased potency, parabens may have an effect at a level that could be seen in humans, Leitman said. |

Tuberculosis (TB) is airborne bacteria disease
Can be fatal if it is not treated early. There were 1454 new cases of TB among Singapore residents last year. A slight rise of 34 from 2013. Most patients are 40 yo or older. Some can be young, three were younger then 10 yo., while another 40 were between 10 to 19 years old. Mr Tan, his doctor, Asso Professor Loo Chian Min said. It is a rare form of disease, affecting 5% of TB patients n Mr Tan's case, it is non-infectious. He caught one from Feb from other infectious one. A full course of TB treatment typically lasts six to nine months. Symptons include a persistant cough lasting more than three weeks, mild fever, weight lost and fatigue. TB is otherwise usually infectious, through close contact with people living or working with, while explaining in his facebook. Adequate shut-eye is needed to clear ' junk' in brain linked to Alzheimer' s, says sleep expert Warning as growing evidence has shown, adequate sleep is necessary to clear the accumulation of " junk" in the brain, which is linked to Alzheimer' s disease, the most common form of dementia that can result in extreme forgetfulness. Prof Chee, director  for Centre for Cognitive Neuroscience at Duke-NUS Graduate Medical School, said that a paper published in 2013 showed that sleep helps clear metabolite - the junk that develops when the brain processes the energy it needs - in adult brains. While doctors are not sure if it is the protein beta amyloid - the metabolite junk which Prof Chee  said causes Alzheimer' s, they do know that there is a huge amount of these proteins in the brains of people with Alzheimer' s. As people get older, their ability to clear this junk from their brain is diminished.Sleep is important in helping to clear away these proteins, he noted, adding that the rate of clearance is about six times during sleep, compared with when one is awake. " If you are sleep-deprived, the rate of clearance of beta amyloids is reduced, so you have more junk floating around in the brain." It is like having a blocked sewage system in the brain, as the sewage piles up, there comes a time when there is so much that it does damage to the brain. Prof Chee said. Pharmaceutical companies have invested billions of dollars trying to produce medicine that can clear beta amyloids from the brain, but have yet to succeed. It takes years of accumulation for damage to be done, typically 10 to 20 years. That is why people in their middle years who do not get enough sleep could find themselves suffering from Alzheimer' s, becoming forgetful by the time they are in their late 50s or 60s. He said. In general, Prof Chee said, people need at least 61/2 to seven hours of sleep a night, with peak performance generally associated with seven hours of sleep. Busy people often think they can get by with four to five hours of sleep a night and that it is all right as they can still manage their daily activities. People who use their brains a lot, he said, " are able to compensate better and they hide it to a point where they cannot hide it any more, then they go bang and get it very bad" ., he said. " By the time you' re diagnosed with Alzheimer' s, it' s too late." But Prof Chee admitted that more studies are needed, as it is still not known if sleeping longer on weekends, for example, can help clear the backlog of beta amyloids and, if so, to what extent. Similarly, is just having seven hours of sleep a night enough, or must it be a deep sleep? Nevertheless, Prof Chee said, it is irrefutable that having enough sleep is critical for good health. Aside from dementia, insufficient sleep is also known to raise the risk of diabetes, some cancers, heart attacks and stroke. According to the CPF Board, these top-ups amounted to about S$500 million last year, up 25% compared to 2013. Singaporean like  former investment analyst Lim Chin Yong, retired in 2012. The 56-year-old has been making voluntary contributions to his own CPF account for the past two years, to earn higher interest on his savings. Last year, he also topped up his wife' s retirement account, so that she can be eligible for the CPF LIFE Scheme. " CPF LIFE to me - at the time when I was looking at it, in 2014 - when you contribute, a minimum of S$148,000, it will guarantee you something like S$1,000 to S$1,200 from 65 until (you turn) 80 or 85, or until you die. S$1,000 to S$1,200 - it doesn' t take you far, so I decided to put money into my wife' s account so that she can have her own income stream of S$1,200 when she retires,&rdquo said Mr Lim. Mr Lim is not alone. According to CPF Board, nearly 74,811 top-ups last year, up 25% compared to 2013. Said Associate Professor Jeremy Goh from the Singapore Management University&rsquo s Lee Kong Chian School of Business: &ldquo When people make decisions like topping up their CPF accounts and looking at how much are the rates that are being paid, they are also looking at what the alternatives are. So if the alternatives out there are not as attractive as the one offered by CPF, then they will choose to top up. When people top up their CPF accounts, it just signals to me at least, that the rates seem pretty attractive.&rdquo If what you want are risk-free returns and a higher annuity payout after retirement, financial advisors say you may be better off putting your spare cash into your CPF accounts. But for those with no spare cash on hand, it might be best to increase your income, experts recommend. Said Mr Tan Siak Lim, financial advisory director at Financial Alliance: " There are not a lot of solutions, to be honest, other than to try to increase their income, because everything starts from your income. If you don' t have excess funds, obviously you' d want all your money to be out, for your daily bread-and-butter necessities." Currently, Singaporeans with sufficient balances in their Retirement Accounts at age 55 will automatically be placed under the CPF LIFE scheme. This is S$60,000 for those turning 55 on 1 January 2016 or after. For those turning 55 between Jan 1, 2013 to Dec 31, 2015, there must be at least S$40,000 in the Retirement Account at 55 years old, or at least S$60,000 in the Retirement Account at payout eligibility age, which is currently at 65. According to the CPF Board website, those with insufficient funds can also apply to join CPF LIFE. |
